About the Role

We wish to recruit a nurse with advanced skills to participate in the community-based Enhanced Health in Care Homes team, consisting of Nurses, Physician Associates, Advanced Clinical Pharmacists and Care Coordinators, working with system partners to deliver the EHCH specification.

Although the post is advertised as full-time, we will happily accept applications from those individuals who are looking for part-time hours.

The successful candidate will participate in weekly multi-disciplinary meetings and will work with the wider team to identify individual care needs and produce proactive individualised care plans for care home residents. The aim is to improve the experience of care home residents, to have a positive impact on primary care service provision, to improve interprofessional relationships, and to reduce hospital admissions.

The post holder will communicate individual care plans with system partners and develop relationships with care homes, community teams, mental health services, palliative care services, social services, and the frailty teams to enable residents to meet their health goals and be cared for in their preferred place of care.

The post is advertised as full time, 37.5 hours per week (part-time applications will be considered).
