Join Our Patient Group on 12th June 2025
Southport and Formby Primary Care Network is holding their patient participation group on Thursday 12th June 2025 between 2-3:30pm at Southport Market.
Everybody that is registered with a GP in Southport and Formby is welcome to join us at the patient group.
We aim to make a difference to primary care (GP practices and community health services) within Southport and Formby and this is your chance to tell us how you feel about our services.
At the event, we will talk about:
- The Primary Care Network (PCN)
- Macmillan Cancer Support Centre
- Enhanced Health in Care Homes and healthy life styles
- Learning disability focus in General Practice
- PCN Pharmacists and antibiotic resistance
- Access to the NHS App
After the meeting, there will be a chance to give feedback and talk to our stall holder partners including:
- Sefton Family Hub
- Mums the Word
- Our Clinical Nurse Lead and student nurses
- Enhanced Health in Care Homes team
- PCN Pharmacist team
- Macmillan Cancer Support Centre
- PCN management team
- Social Prescribing Link Workers
