Patient Safety Incident Response
Patient Safety Incident Response Framework:
Organisations who hold NHS contracts are required to develop and maintain effective systems and processes for responding to patient safety incidents for the purpose of learning and improvement in patient safety in healthcare.
The approach supports the four key elements of the national Patient Safety Incident Response Framework to provide:
- compassionate engagement and involvement of those affected by patient safety incidents
- a considered and proportionate response to each patient safety incident and safety issue
- the application of a range of system-based approaches to learn from patient safety incidents, and
- a supportive oversight, focused on strengthening response systems to facilitate improvement
